I think it’s actually two or three fish, like some kind of carp. Possibly koi.

Or simply a dragon.

@lady-asuka In the art you found, the design has long whiskers, or barbels, like certain types of fish, like some goldfish, koi, and carp.

I really do think his tattoo is two or three fish swimming around his arm.

Whatever the design is, there is more than one. There are at least two heads at different places: one at his shoulder, and one on his arm. There might be a third.

I cannot be absolutely 💯 sure, but this looks like very stylized fish to me. Not one, but at least two. I see more than one head and space between them.
